چکیده

H ere, we describe a 52-year-old woman with a history of primary plasma cell leukemia (PCL) who achieved complete remission after the tandem of autologous and nonmyeloablative allogeneic stem cell transplantation (SCT). Nine months after the allogeneic SCT, she presented with oculomotor and trochlear nerve palsy (diplopia, ptosis of the right upper eye lid, and dilated right pupil). Physical examination also revealed mild paresis of the upper arm muscles and the muscles involved in dorsiflexion of the left foot. Contrast-enhanced craniospinal magnetic resonance imaging showed multiple enhancing leptomeningeal nodules (panel A arrows). Cerebrospinal fluid (CSF) examination revealed 321 3 10 leukocytes per L and an elevated total protein level of 2.08 g/L. Microscopic analysis of the CSF showed the presence of very atypical plasma cells (panel B), some of which contain intranuclear inclusions (Dutcher bodies). Immunophenotypic analysis confirmed the presence of plasma cells in the CSF (60% of all nucleated cells). These cells had an aberrant phenotype (CD38 CD138 CD19 CD56 CD45; panel C). There was no systemic activity of the primary PCL.

Leukemia and Lymphoma Metastases

Central nervous system (CNS) metastases can occur with any primary systemic cancer, but some primary cancers such as melanoma have a specific predilection for the CNS. Brain metastasis is the most common CNS metastasis, occurring in 15% of all cancer patients (Posner, 1995). Leptomeningeal metastasis is less common, 3% to 8%, and epidural metastasis occurs in approximately 5% of cases (Posner, ...

Leptomeningeal Metastases in a Patient with an Extragonadal Germ Cell Tumor

We present a case of leptomeningeal metastases in a 30-year-old man with an extragonadal germ cell tumor. The patient was referred to our hospital for treatment of an occipital brain metastasis. This lesion was resected, followed by whole brain radiotherapy and further chemotherapy, and a temporary complete remission was achieved. However, leptomeningeal recurrence developed, and despite salvag...

Intracranial Leptomeningeal Carcinomatosis from Breast Cancer Detected on 18F-FDG PET.

Leptomeningeal carcinomatosis is an uncommon manifestation of non-central nervous system (CNS) metastatic disease. Diagnosis, however, has important prognostic and treatment implications. We present a case in which intracranial leptomeningeal carcinomatosis from a primary breast cancer was detected with (18)F-FDG PET/CT, despite its low sensitivity for detection of CNS metastases from non-CNS p...

Constitutive integrin activation on tumor cells contributes to progression of leptomeningeal metastases.

Leptomeningeal metastases are a serious neurological complication in cancer patients and associated with a dismal prognosis. Tumor cells that enter the subarachnoid space adhere to the leptomeninges and form tumor deposits. It is largely unknown which adhesion molecules mediate tumor cell adhesion to leptomeninges. We studied the role of integrin expression and activation in the progression of ...

Hodgkin’s Lymphoma Occurring Secondary to Autologous Stem Cell Transplantation in Plasma Cell Leukemia; A Case Report

Survival of patients with multiple myeloma has improved substantially because of availability of new therapies including autotransplants, immunomodulating drugs and proteasome-inhibitors. Second primary cancers have emerged as an important determinant of morbidity and mortality among cancer survivors. Even though there is an increased risk of new cancers of the lymphoreticular and haematopoetic...
